Battle of the Hogs Release Form 
I, the undersigned, individually and as parent(s) and / or guardian(s) of 
_________________________ a minor, ask that he/ she be admitted to 
participate in this sport competition. In association of such admission, I do herby 
release, discharge, and hold harmless the North Penn Valley Boys and Girls 
Club and Lansdale Catholic High School, it’s officers, agents, volunteers and 
employees of and from all causes, liabilities, damages, claims, or demands 
whatsoever on account of any injury or accident involving the said minor arising 
out of the minors attendance at the sport competition and / or activities held in 
connection with the sport camp.

The entry fee is $25.00 per person equaling to $100.00 per team. Make all
checks payable to the North Penn Valley Boys & Girls Club. Please send
check or money order to North Penn Valley Boys & Girls Club 16 Susquehanna
Ave. PO Box 103 Lansdale, PA 19446. Please do not send cash and sorry, we
do not accept credit cards. You will be notified of acceptance and sent a receipt
by email.
